NDIS Plan Manager
As we are a NDIS Registered business, we are providing NDIS Plan Manager services to our valued customers. We can help you to manage your NDIS budget. We take care of managing the financial side of your plan. So you have more freedom and flexibility. Our expertise and experience in the industry will surely help you to get this done much better than ever.
Domestic Daily Activites
Our service range includes assistance with daily living as well. At Community Support and Care, we provide support for regular personal activities and personal daily tasks. We cover both Core Support and Capacity Building Support.Core Supports includes daily tasks such as bathing, dressing, gardening, shopping etc. Capacity Buiding Support includes activities to build independence skill development.
Personal Care
Bathing, showering, dressing and undressing, grooming, toileting, eating, meal preparation/delivery.
PARTICIPATION IN COMMUNITY, SOCIAL AND CIVIC ACTIVITIES
Developing a client’s ability to partake in social, community and civic activities and supporting them during the activities.
LIFE-SKILLS
Development of daily living and life skills to increase the client’s ability to live as autonomously as possible.
PHYSICAL WELL-BEING ACTIVITIES
Provision of physical activity, nutritious diets.
HOUSEHOLD TASKS
The aim of the program is to ensure a safe, secure, healthy home environment to enable frail older people, people with dementia and younger people with disabilities (who are eligible through the National Disability Insurance Scheme- NDIS) to live independently at home and in their own community. House cleaning is also provided to the care recipient’s level of physical functioning and to suit their cultural preferences. In addition, we help with washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to individuals care needs and transport to help with shopping, visit the doctor or attend social activities
COMPANIONSHIP
We offer more than just physical assistance – our care recipients will have someone to talk to, someone to listen. Our staff offer caring, compassionate, thoughtful conversation and companionship.
TRANSPORTATION
We offer transport to medical appointments, shopping, social outings and family events ensuring that our client gets there in a safe and timely manner.
ACCOMMODATION/TENANCY ASSISTANCE
Assistance to ensure that our clients obtain/retain safe and appropriate accommodation.
RESPITE CARE
This service seeks to reduce the emotional and physical stress on cares and can be offered in the person’s home with care ranging from a few hours each week to overnight care (in-home respite).
Supported Independent Living (SIL)
Supported Independent Living (SIL) is help with and/or supervising of daily tasks in a shared living environment, with a focus on developing the skills of each individual to live as independently as possible. The support is provided to each person living in the shared arrangement based on their support needs. Assistance provided to a participant will be included as part of their plan depending on the level of support they require to live independently in the housing option of their choice. SIL funding is typically for people who need 24/7 support while living in a shared home. It exists to cover the cost of the support staff in the house. The most important part of the Supported Independent Living puzzle is a Support Coordinator. They’re paid by the NDIS to help you navigate the process and liaise directly with allied health and service providers. They will often do a lot of legwork for families.
The above services will be offered on a fee-for-service basis and/or subsidized by a variety of Government programs, including NDIS.

All carers are fully employed by Community Support and Care Pty Ltd and we avoid using temps or supply agency staff. Every employee holds a recognized qualification.
